어느덧 35세가 된 실비아(Emmanuelle: 실비아 크리스텔 분)는 얼굴 뿐만 아니라 몸까지 20대처럼 젊고 탄력있게 해주는 성형수술을 하고 제2의 엠마뉴엘(Emmanuelle IV: 미아 니그렌 분)로 탄생한다. 새로운 육체의 적응을 위해 여의사 도냐(Dona: 데보라 파워 분)와 함께 여행을 떠난 엠마뉴엘은 여러 도시를 전전하면서 연애 행각을 벌인다. 리오에서 그녀는 자신이 묵는 집의 여주인과 정사를 나누고, 도냐 역시 그녀에게 빠져든다. 그리고 엠마뉴엘은 리오에서 옛 애인 마크(Marc: 패트릭 보초 분)를 만난다. 그러나 그는 전혀 엠마뉴엘을 알아보지 못한다. 엠마뉴엘은 마크를 한눈에 사로잡고는 그냥 스쳐지나가 버린다. 말크는 엠마뉴엘을 못잊어 그녀에게 선물을 보내고 다시 두 사람은 새로운 사랑을 시작한다.

With this new identity comes a sexual awakening which is complicated by her memories of Marc. As with other entries in this series, the plot twists provide ample opportunities to expose the characters.
